Proof of Address
A proof of address is a document that confirms your residential address. For a POA document to be accepted, it must contain this information:
- Your name as per NRIC/Passport
- Your residential address
- The date the document was issued
- The name and logo of the issuer (company/organization)
Types of acceptable POA documents
- Bank Statement
- Utility Bill (internet, electricity, water, sewerage, etc)
- Cable Bill (Astro, etc)
- Cell Phone Bill (Celcom, Digi, Maxis, UMobile, etc)
The requirements for valid POA include:
| Name and address | Your name and address on the bill must match the name and address used when registering your Tokenize Account. |
| Date |
Must be dated within the last three (3 months). e.g.: POA submission is 7th April 2021. Accepted POA must be dated between 7th January to 7th April only. |
| Other | The document uploaded must be a full page, coloured scan. |
