Proof of Address
A proof of address is a document that confirms your residential address. For a POA document to be accepted, it must contain this information:
- Your name as per NRIC/Passport
- Your residential address
- The date the document was issued
- The name and logo of the issuer (company/organization)
Types of acceptable POA documents
- Utility bill (internet, electric, water, gas, sewerage (Indah water) bill),
- Cable bill (Astro, etc),
- Cell phone bill (Celcom, Digi, Maxis, UMobile, etc),
- Internet service provider account statement,
- Bank letter or statement (All financial products issued by local Financial Institution, example saving account, credit card, etc),
- Mortgage statement, (Mortgage statement issued by the local bank or other local registered lending institutions)
- Municipal rates & taxes, (Statement from local municipality showing rates & taxes relevant to the property)
- Declaration issued by authorised authority (signed and stamped with a government-issued stamp),
- Investment account statement (Issued by local licensed entity such as Kumpulan Wang Simpanan Pekerja, Tabung Haji)
- Insurance statement (issued less than 12 months),
- Lease or Rental Agreement (issued less than 12 months with LHDN Stamp duty)
Note: Acceptable only for documents issued by local registered entities in Malaysia.
The requirements for valid POA include:
| Name and address | Your name and address on the bill must match the name and address used when registering your Tokenize Account. |

- Must be dated within the last 3 months. e.g.: POA submission is 7th March 2024. Accepted POA must be dated between January to March only. - Less than 12 months for Insurance Statement and Lease or Rental Agreement |
| Other | The document uploaded must be a full page, coloured scan. |
